Here you will find all the forms you might need. You know, if you spot something odd in one of these forms, or maybe think of something we should add, or perhaps you’re really good at finding spelling and grammar errors, then we would love to hear from you so we can update and improve these forms.  That would be very valuable feedback for us. Chicken Adoption:
If you are keen to adopt some chickens you will need to complete this form: Chicken Adoption form inquiry.
Send it back to info@newlifeforanimals.org.au and the committee will confirm that you have been approved for adoption. If there are any problems with your application someone will contact you to discuss the matter and help fix up any details.

Once you’ve been approved to adopt chickens a time will be arranged for them to be brought to you. At that time you will need to complete the Chicken adoption agreement form.  We take this agreement very seriously and we do follow up to ensure it is being upheld. Please make sure you are familiar with the terms of the agreement.

Surrender:
If you are no longer able to care for any of the animals we can accept them in to one of our safe foster carer situations. You will need to complete an Animal Surrender Form. We know this is a tough decision for you to make and we would like to support you in this process. We constantly hope never to have to use this form but sometimes it is just unavoidable, so we promise to give your animals all the love and attention that you would like to give them. We will ensure that any home they get resettled in would be considered perfect, safe and full of love. Nothing less will do.

Taking on animals for people when they can no longer care for them is expensive and time consuming, however, we usually feel we can’t insist on a financial contribution because that risks having the people look to euthanasia or even dumping animals as a result. If you are surrendering an animal please take a moment to think about making a donation to go towards the care of the animals.

Foster Care:
To become a foster carer is a noble action, and we desperately need people to foster animals. We have roles for foster carers with chickens, dogs, cats, rabbits and doves. We could also use some help with farm animal foster carers such as piglets, lambs and goats.
